I eventually took the Pajero to DupDiesel in Parow and the vehicle was fixed. There was nothing wrong with the cylinder head, compression, injectors, hoses etc. The head man at Dup explained that there is microscopic wear in the fuel pump that causes a loss of pressure in the fuel lines meaning that fuel runs back down the lines when the engine/pump is cold and the metalhas contracted. The vehicle will start normally as there is a bit of fuel in the pump but when this runs out the fuel that is being pumped up the lines does not get a chance to reach the pump as the pumps ability to pump when cold is reduced by the wear. This is why you literally have to crank the motor until the battery is almost flat to start it. The pump simply cannot pressurise the system.
However, when the engine is hot the various metals expand in the pump and then everything is ok as there is pressure and starting is no problem. The expansion cancels out the microscopic wear.
It cost me R4675.00 to have the pump rebuilt and she runs and starts like a dream again.

By the way, if anyone is interested, she will be for sale in mid April.
