Hole in Piston - Pajero DID

Has their been anyone that has experienced a round hole in a piston
It would be great to isolate the cause

Their are no objects or broken pieces in the engine or piston chamber

The vehicle in question is a 2002 Pajero Diesel DiD with 185000 km. No previous problems
engine has not been opened before, no pieces of metal or any other foreign objects found in the Engine after removing the cyl head. Injectors and glow plugs in order ie. not damaged or pieces missing.

If you say injectors in 'order'. does that mean it has been tested for the correct spray pattern? A hole in the piston as you describe is normally due to incorrect spray pattern from the injector leading to extreme heat hot spots. If it is found that the injectors are at fault maybe replace all the pistons as they might all have been subjected to the same heat and another one is bound to take the same route south as the one that you mention.
