3.2 Diesel 'missing' over 3000 revs?

For the last few weeks I felt a bit of a 'miss'/hesitation when pushed past 3000 r/min, which is not often, on the 3.2Did diesel. She idles fine and starts first swing with no more smoke than any other diesel engine with 275000km on.
This past weekend in Lesotho at altitude over 3000m on a steep pass this was particularly prevalent but it still did this on the way home and revved past 3000. The engine seem to missfire(impossible on diesel as far as I know) and hesitate slightly and loose power.
I have searched a bit on the net and it seems that it can be a million and one things from clogged diesel filter, open stuck EGR valve, sticky injector, turbo overboost, speed sensors on the diesel pump and off course the diesel pump itself!!
I recently replaced the intercooler myself and seem to think that this missing started somewhere after that. Maybe related in some way?
Question is - where do I start to resolve this issue in order of preference and least cost? Any pointers please?

Hey Cats, Thats a bit of a worry. when did you last have your injectors tested? have you done a fault code read out? you can rule out the EGR, it would only give you problems at idle and I reckon if you had pump issues you'd probably have smoke too.

Diesel engines are basically very simple in operation (until they add electronic controls, emission reducing bits etc).
If it misfires, it is most probably fuel supply problems causing it.
The following are the obvious items to check:
- Fuel quality (eg water)
- Fuel filter blocked
- Diesel pump (specialist required
) - Injectors
I don't know whether I should ask, but do you run 2-stroke oil in your diesel? You should (at 1:200)

Injectors was checked about 15000km ago IIRC and deemed to be fit, but this is probably not to say that one got stuffed in the mean time due to bad fuel etc?

Lol - no I am one of the non believers. I put in diesel and drive. Oil every 5000km I do believe in. Why I dont know?
I had a look this evening and the airfilter is disgustingly dirty and dusty! It is just about time for a service so will replace ALL filters over the weekend. Maybe (thumbs crossed) she just need a good full service. Will also get some of that official Mitsu snake juice and run through the system to see if that clears any possible injector problems.

Yip CATS,
Absolutely the place to start - good service and new filters all round. A dirty air cleaner on a TD motor isn't clever at all, pushes up the temp as it starts to over fuel, but most likely not the cause of your symptoms. My money is on diesel delivery and hopefully solved with a nice clean filter. A bad tank of diesel can go a long way towards ruining a diesel filter.
Hope it gets sorted soon - you have a mountain to climb on the 29th Sep

OK feedback time.
The problem seem to have misteriously dissapeared? Maybe it was linked to the tank diesel I had in at the time. Remember I have longrange tank so it takes long to drive out about 130litres, unlike in a petrol monster like Roelf.
Anyway I had the 275000km service done, I still do every 5000km, so even though this was a small service I requested new diesel filter as well. Will keep an eye open, but so far she is purring to go mountain climbing in September with Roelf and his 'Rock and Roll' tour!
Unfortunately the srevice also showed up a popped rear inner cv boot, so will have this fixed when I get the part from Mitsu.
