Erratic idling and smoking

Good morning,
I have a 2004 , 3.2 DID SWB which has served me well. The present odometer reading is 478000 km so it is well travelled. I have maintained service levels throughout and expected wear and tear has been addressed. A little while back I started experiencing excessive smoking particularly when running the air conditioner and/or idling at stop signs and traffic lights. Starting was also becoming more difficult and idling was rougher than before. However cruising on the open road did not seem to be affected and only stop/start driving was problematic. I took the vehicle in to a local workshop and after investigation and diagnosis, the glow plugs were replaced and injectors removed and serviced. Since then starting is much better but the rough idle and smoking still persist. What is strange to me is that when the vehicle idles in park, it is quiet and no smoke. As soon as reverse or drive are engaged, the idle noise and smoking start again, even whilst the vehicle is stationary. Once mobile, the problems seem to desist and she runs beautifully, particularly on longer trips. Has anyone else had a similar experience or perhaps able to offer some advice on where the problem might lie ? I am planning to get a second opinion from another workshop, but would also be thankful if any of the experts in the club can provide some input.

Yes, I am 95 % sure that it only happens once the temperature is up, but I would need to check on this tomorrow to eliminate any doubt.

Checked this morning and it also does it after starting and the engine still cold but not as bad. Almost seems to be "jibbing" when reverse engaged as if idling too slow. Is there an idling adjustment on the engine ?
